Make Sure Enrollment Forms Are Complete And Legible

Please check all 457 Plan enrollment forms closely before you fax them to Voya Institutional Plan Services, LLC at 1-888-228-6185.

If information is missing or you can’t read what employees wrote, contact the employee to confirm the name, address and Social Security Number. If the form needs to be redone, ask the employee to print neatly and sign it.

When you fill out the employer’s section, spell out the employer name completely (no initials or abbreviations). Write in your Agency Plan Number so the participant gets enrolled in the right plan. Be sure the form has the employer’s and participant’s signatures.

Whenever you have questions or need help with enrollment forms, please contact your plan administrator at CalPERS_Plan_Admin@voya.com.

June 2 Webinar On Staying The Course

Your Shortcut For Updating Intranet Pages

If your agency has intranet pages regarding the CalPERS Supplemental Income 457 Plan, use the 457 Plan’s employer intranet template for automatic updates.

The employer intranet template includes the latest information about the 457 Plan you can cut and paste into your intranet pages. Live links in the copy allow your employees to schedule one-on-one appointments with a CalPERS 457 Account Manager, log into their accounts or download forms.

Click here or go to calpers-sip.com and select Employer Intranet Template.
